چکیده

A 71-year-old-man was admitted to our hospital with a fever over 39°C. He had undergone pancreaticoduodenectomy for cholangiocarcinoma and suffered from recurrent cholangitis for a year after the surgery. Although laboratory data showed an elevated white blood cell count (13,100/mL) and C-reactive protein (6.53 mg/dL), no marked elevation was noted in the hepatobiliary enzymes. However, abdominal magnetic resonance imaging revealed the dilatation of the intrahepatic bile ducts and a high-intensity area on T2and diffusion-weighted imaging in segments 4/8. A liver abscess was suspected. Abdominal computed tomography showed a linear opacity with hyperattenuation in the intrahepatic bile duct and a stent in the pancreatic duct that had been inserted during pancreaticoduodenectomy (Figure 1). Subsequent double-balloon endoscopy revealed foreign bodies in the orifice of the right hepatic duct (Figure 2). Those foreign bodies were found to be fish bones (Figure 3). The fish bones were removed from the bile duct, and the patient’s symptoms resolved. We used a modified Child’s method for reconstruction. Although the migration of fish bones into the common bile duct as a cause of

Papillary carcinoma of the duodenum combined with right renal carcinoma: a case report

We report a case of papillary carcinoma of the duodenum combined with right renal carcinoma. A 58-year-old man underwent a physical examination that revealed intrahepatic and extrahepatic bile duct dilatation on B ultrasound. Intrahepatic bile duct dilatation could be seen on magnetic resonance imaging (MRI), but the head of the pancreas and distal bile duct showed no tumor signals, which led t...

Direct colonoscopic visualization of intrahepatic bile ducts in a post-pancreaticoduodenectomy patient

A 79-year-old female was evaluated in consultation for painless jaundice. Past medical history was signifi cant for obstructive jaundice secondary to distal bile duct stricture from a benign pancreatic head mass status postpancreaticoduodenectomy. She later developed recurrent cholangitis and during surgical exploration was found to have a left hepatic duct bifurcation stricture that was resect...

Squamous cell carcinoma of the distal common bile duct.

CONTEXT Squamous cell carcinoma of the biliary tree is rare. Although few cases of squamous cell carcinoma of the intrahepatic bile-duct and gallbladder have been reported, until today, only four cases of squamous cell carcinoma of the extrahepatic bile duct have been reported in the literature. CASE REPORT We present a case of squamous cell carcinoma of the distal common bile duct presenting...

Metachronous bile duct cancer in a patient surviving for a decade and undergoing curative surgery twice.

We report a 75-year-old woman with metachronous bile duct cancer who underwent curative resection twice and has survived for a decade. In 1989, she was admitted because her serum alkaline phosphatase level was elevated. Computed tomography (CT) showed a low-density mass, 2 cm in diameter, at the left hepatic duct and intrahepatic bile duct dilatation in the left lobe. We diagnosed the lesion as...
